If you would like to download the form now and fill it out ahead of time, that would certainly save you time when you arrive for your appointment. Just click the link below and then print out the form on your printer. Fill it out at your convenience and bring it with you at the time of your appointment. Or, you can drop it off anytime prior to your appointment as well.
Behavior Questionnaires
If you have a behavior consultation appointment set up with Dr. Boucher, please download the appropriate file (dog or cat), fill it out and drop it off at the hospital PRIOR to your appointment. If you can bring it in a few days before the appointment, that would be ideal. The more time Dr. Boucher has to evaluate the history, the more effective the appointment time can be. Thank you.
